We usually assume the arrow points one way: anxiety upsets the stomach. But it also runs the other direction — an upset stomach can genuinely feed anxiety. The gut sends far more signals up to the brain than it receives back, so what happens in your gut can shape how safe or on-edge you feel.
The gut talks up to the brain
The vagus nerve — the main data cable of the gut-brain axis — carries roughly four times as much traffic from the gut to the brain as the other way. Gut discomfort, inflammation, distension and even the mix of microbes present all generate signals that reach brain regions involved in mood and threat detection.[1][2] Your brain reads these bodily signals and folds them into your emotional state — a process scientists call interoception. A churning gut can, quite literally, be interpreted as unease.
Why this becomes a loop
Because the traffic is bidirectional, an upset stomach and anxiety can reinforce each other: discomfort raises anxiety, anxiety tightens and sensitises the gut, and round it goes. This is the mechanism behind conditions officially reclassified as disorders of gut-brain interaction, where gut and brain signalling are turned up together (see IBS and the gut-brain axis).
What the microbiome adds
The gut's microbes shape these upward signals. In animals, altering the microbiome changes both gut function and anxiety-like behaviour, and colonising germ-free animals within a developmental window normalises an over-reactive stress response.[3] In people the picture is subtler and still emerging, but the microbiome is clearly part of the volume control on gut-to-brain traffic.
What this means for you
If your anxiety tends to spike alongside gut symptoms, treating the two as one connected system — rather than "all in your head" — is both accurate and useful. Steadying the gut with fibre, fermented foods, sleep and stress-recovery can help lower the baseline noise. Persistent gut symptoms or anxiety deserve professional support.
